Waver

For insurance purposes we require all guests at the studio to sign a waver form. 

LIABILITY WAIVER FORM

I acknowledge that I am using the facilities at the West Coast Photo Studio and that my activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By stating YES in the form, I assume all risks related to the user of any and all spaces used during the photo session at West Coast Photo Studio.

I agree to release and hold harmless, Bob Garlick of DB Communications and Garlick Photography and the West Coast Photo Studio including their agents and
assigns from any cause of action, claims, or demands now and in the future. I will not hold liable the people mentioned above and facilities for any personal injury or any personal property damage, which may occur on the premises before, during or after photo sessions.

Furthermore, I agree to obey studio rules and take full responsibility for my behavior in addition to any damage I may cause to the facilities and equipment utilized by Bob Garlick of DB Communications and Garlick Photography the West Coast Photo Studio.

I understand that Bob Garlick of DB Communications and Garlick Photography and the West Coast Photo Studio are licensed, accredited and are insured organizations.

If I should observe any unsafe conduct or conditions before, during or after my use of the space, I agree to report the unsafe behavior or conditions to Mr. Bob Garlick or a staff member at the West Coast Photo Studio as soon as possible.

I agree to abide by this Liability Waiver and affirm that I have read and are in agreement with the contents of this waiver by choosing YES in this form.
